This waiver is valid for all yoga classes conducted by the Unitarian Universalist Congregation in the Assabet Valley from October 11 th , 2026 through October 10 th, 2027.

I, the above named, on the listed date, agree
with UUCAV to the following:

I agree I am willingly participating in yoga class(es) provided by and hosted by the Unitarian
Universalist Congregation in the Assabet Valley. I understand that participation in yoga class is
contingent upon the signing of this waiver.

I release UUCAV and its representatives, employees, or volunteers from any responsibility or liability concerning the application, processes, and or consequences of the yoga class in which I have elected to participate.

I acknowledge that yoga is a physical activity which may require exertion and carries inherent risk of injury. I agree that I am of sound physical health and that it is my responsibility to consult with a medical professional before participating in yoga class. I represent and warrant that I have no injuries, illnesses, or conditions which would prevent me from full participation. I am
aware that participation in a group class setting may result in exposure to infectious agents including COVID 19 and I voluntarily assume that risk.

I agree that the health information shared in class is the opinion of the yoga instructor and should not be used as a substitute for professional medical advice, diagnosis, or treatment.

I agree to assume full responsibility and waive any claim I may have against UUCAV for injury, infections, or damages I may sustain as a result of participation in yoga class.

I, my heirs, and my legal representatives release, indemnify, waive, and forever discharge UUCAV, its employees, representatives, agents, officers, and volunteers from all manner of actions, liability, claims, causes of actions, and demands arising out of or related to any loss, damage, or injury to my person or property, infections, or death caused by their negligence, other acts, or by my actions while participating in yoga class(es).

I understand that UUCAV is not responsible for the loss, theft, or damage of any personal possessions that I bring to class or to UUCAV property.

I have fully read and understand the contents of this waiver and agree with the above and sign freely and of my own volition. I understand that I am giving up rights, including the right to sue.
